The incubation period of HAV averages 28 days (range 15 to 50 days). Hepatitis A is usually a self – limited illness that does not become chronic. Most adults with HAV infection have symptomatic illness which begins with abrupt onset of nausea, anorexia, fever, malaise, and abdominal pain. Within a few days to a week, dark urine and pale stools appear, followed by jaundice and pruritus. The early clinical manifestations usually diminish when jaundice appears, and jaundice typically peaks within two weeks. The laboratory abnormalities include abnormal liver function tests. The liver enzymes peak approximately one month after exposure to the virus and then decline by approximately 75 percent per week. The serum bilirubin concentration usually declines within two weeks of peak levels. The diagnosis of acute HAV infection should be suspected in patients with abrupt onset of gastrointestinal signs and symptoms and jaundice or elevated liver blood tests. The diagnosis is established by detection of serum immunoglobulin IgM anti – HAV antibodies. The treatment consists of supportive care. Full clinical and biochemical recovery are observed within two to three months in most patients, and complete recovery is observed by six months in nearly all patients. HAV infection confers lifelong immunity. Prior to exposure, the primary tool for protection is vaccination. The preventive methods include handwashing (including after using the bathroom, changing diapers, and before preparing or eating foods), heating foods appropriately, avoiding tap water and raw foods in areas with poor sanitation.
